Yes, you will need to provide your name, your 3-4 ID, and your date of birth to authenticate yourself as an HCA Healthcare employee.

Your 3-4 ID is a combination of three letters and four numbers specifically assigned to you (example:  ABC1234).

• It is used to access many HCA Healthcare systems.
• This is not the same as your Employee Identification Number (EID), which is a 9-digit number.
• You will need your 3-4 ID to verify your eligibility to use the Remotiv benefit.
• If you don't know your 3-4 ID, contact your manager or call HCAGHR at (844) 472-6797 for assistance.
